Preface:

This book was written as a sequel to "Vitamin and Mineral Requirements in Preterm Infants" (1984) and "Nutrient Requirements in Preterm Infants" (1993). In all three instances, an education grant froM Mead Johnson Bristol-Myers Squibb was essential to bring the authors together for a full two days of interaction and mutual review. This integrated team approach has been a critical reason we believe why this book series has stood the test of time and value. The authors gathered in Chicago this time at O'Hare Airport. We wasted little time, but quickly engaged in lively constructive discussions, honing in on how each nutrient requirement was derived, and the advances of the last decade. Much has transpired in the last two decades in terms of understanding nutrient importance for optimal survival, especially in the increasingly smaller preterm infants we now take care of in the nursery. A great deal of attention was focused on practical issues of nutrient delivery, and the special circumstances of specific groups of preterm infants.

Many manuscripts were mailed ahead of time by authors for preview, except for a few who were still editing their manuscripts on site on their laptop computers. Indeed many discussions prompted instant revision of the laptop-based manuscripts; such is the nature of modern manuscript writing. After thorough review and vetting, all papers were again sent to a web-based repository of manuscripts for access and change, on the web, by selected "internal" (ie, other book authors) and "external" (non-authors) reviewers. All joined in the fray with good vigor (and humor in some instances). The process has been long and arduous, with many complications (illness, lost mail, changes in jobs, etc.), but finally we are here!

The present editorial team is a seasoned team. Ricardo Uauy and Stan Zlotkin were former co-editors, and Bert Koletzko was a thoroughly involved author of the previous edition. We instantly "bonded" again and moved to perfect this book, which many have told us is the "bible" for nutrient requirements for preterm infants. The enthusiasm and patience of the team have been inspirational.

Throughout all of this, our publisher, who had been similarly involved in previous books with the Editors (especially Nutrition in Infancy), has been thoroughly engaged. A veterinarian by profession (now a fish doctor he tells me), and publisher/biomedical communications entrepreneur by "hobby", it has been a good and fruitful partnership with Tim Mullican, DVM.

One touch omitted from this edition is any royal endorsements! Anne, HRH the Princess Royal of the British Isles, wrote in the last edition "the whole process ... was conducted with good grace and remarkable little acrimony, - a commendable (and surprising!) achievement for medical experts. I would expect this book to ne most influential and warmly received and to set a high and hopefully consistent, international standard of nutritional care for premature infants." We certainly hope the present edition lives up to its previous reputation and HRH Princess Anne's expectations, to bring us to another level of excellence.
