NOTE: The forms generally used by the Court system in some instances are specific documents actually supplied by the Court system. For example, the complaint form for a commercial case in the NYC small claims court (FORM 2) is a one-page (double-sided) yellow form.
The same form for a non-commercial case (FORM 1) is a one-sided white page form with red print. The information subpoena form for NYC cases, (Form 21) is a multi-page form with carbons that can only be filled out at the Clerk's office. The forms supplied by the Court are the forms that should be used and not the forms in the book.
The book contains examples of the most frequently used forms to provide an understanding where and when these forms are used and how to fill them out. Most important, these forms will give you the knowledge of what forms to ask for when you are in the Court building.

About the Author
Richard A. Solomon is an author, lecturer, and trial attorney located in New York.
In April 2001, Mr. Solomon was asked by Dean Everett Bellamy to create and teach
a class on small business law at the Georgetown University Law Center.
Mr. Solomon has also lectured on the topic of civil discovery for the Florida Law Enforcement Community in Miami.
He earned his J.D. degree from the Georgetown University Law Center and graduated Phi Beta Kappa with high honors from the State University of New York at Binghamton.
